Evergreen Nephrology is a nephrology focused value-based care organization that works with nephrology practices to deliver value-based care for patients with kidney disease. They enable nephrology practices to take risk on their Medicare populations and provide services to enable VBC performance, including interdisciplinary clinical teams that support patients in the home and tools to support care management activities.
