ACO Enablers and Primary Care Aggregators
Enablers and aggregators in this category work with physicians and provider practices to transition to value-based care, supporting end-to-end from contracting through operations. These entities aggregate practices through structures like ACOs (Accountable Care Organizations), CINs (Clinically Integrated Networks), IPAs (Independent Physician Associations), and/or MSOs (Management Services Organizations). They help practices access financial benefits from value-based contracts including Medicare's value-based care programs, MSSP (the Medicare Shared Savings Program) and ACO Reach (Realizing Equity, Access, and Community Health) Model. They may also contract directly with other payers, such as commercial health insurance plans or managed Medicaid plans, on behalf of providers in their networks. They generally provide robust technology, tools, and services to enable practices to succeed in VBC.
Criteria:
• Practice aggregation through ACOs, CINs, MSOs, and IPAs
• Risk-bearing capabilities to take risk on behalf of practices
• Typically Medicare-focused, oriented around Medicare's VBC programs. Some contract directly with private health insurers to cover Medicare Advantage, Medicaid and Commercial populations
• Physician enablement capabilities such as data, analytics and services.
